Exceptional Family Member Program

The Exceptional Family Member Program (EFMP) is a coordinated effort to support military families that have at least one family member with exceptional needs by providing information and community services referrals, educate families on community resources and assists families with accessing them. Resource information to include: national & local services, early intervention & child care options, offers recreational and educational events, Respite Care Information/Options, conducts Family Needs Assessments & Family Service Plans (DD 3054), Relocation Assistance and School Information on Special Education.
 

Under the umbrella of the EFMP you have:

  • EFMP-FS (Family Support)

The Family Support function provides information, referrals, parent education, and recreational events.
All three functions work together and with other military and civilian agencies to ensure EFMP families have an enhanced quality of life as they relocate around the world with their sponsors.

What is EFMP?

How To Enroll In EFM?

All EFMs complete a DD Form 2792. In addition, guardians of minors complete DP Form 2792-1. "Google" these forms online or pick them up at any of the 87th Medical Group EFMP One-Stop Offices.

Military Onesource is Now Available to Veterns and their Families

A Full Year after Separating from the Military

Washington, D.C., (Aug. 13, 2018) —The Department of Defense (DoD) announced extended eligibility for Military OneSource benefits from the current 180 days to 365 days after separation or retirement from military service to ensure all service members and families have access to comprehensive support as they transition to civilian life. This change goes into effect 13 Aug 18 in accordance with the John S. McCain National Defense Authorization Act for fiscal year 2019. As a DoD program, Military OneSource offers a wide range of services designed exclusively for the military community. Services include help with relocation, tax support, financial planning, health and wellness coaching, as well as confidential non-medical counseling and specialty consultations for spouse employment, education, adoption, elder care, special needs and much more.

EFMP One - Stop

Located within the 87th Medical Group Clinic. In addition to the EFMP-Family Support Coordinator, the EFMP One-Stop provides:

SPAN’s Military Family 360 Support Center is a one-stop center assisting military families — active duty and retired military and National Guard and Reserve personnel throughout New Jersey, who have infants, toddlers, children, and youth ages birth to 26 with disabilities. Military Family Support 360 assists military families by: developing a family plan to address concerns; assisting with access to family resources; offering training sessions on advocacy for accessing quality services, health care, education; offering family to family emotional support and information; linking you to leadership opportunities to ensure the family voice in decisions about military assistance programs and services.

Who is EFM?

An Exceptional Family Member is defined as one, regardless of age, who has a disability that limits the ability to function on a daily basis and required ongoing counseling, training, education, therapy and treatment. Some examples of disabilities include: Asthma, Autism, Emotional Disturbance, Hearing Impairment, Mental Health Disorders, Multiple Disabilities, Orthopedic Impairments, Specific Learning Disabilities, Speech or Language Impairment, Traumatic Brain Injury and Visual Impairment.

EFMP & ME

An online tool for military families with special needs; offering 24/7 access to information that is tailored to individual needs through checklists. These checklists provide steps, resources, and tips to navigate through topics such as:

  • Preparing for a PCS or Deployment
  • Navigating special educational and medical needs
  • Getting organized and enrolled in EFMP

EFMP - Medical

Enrolls sponsors in EFMP and manages enrollment forms, submits Q-Code letters to MPF for updated personnel files, contacts EFMP families at least annually for updates, submit DD-2792 changes and updates health changes, processes all PCS clearances, CONUS and OCONUS, EFMP verification for Tricare ECHO enrollment, and assist with referral management and medical coordination.

School Liaison Officer

Military families who move to the Joint Base can meet with the school liaison specialists to discuss the schools and district options available to them. Develops and coordinates partnerships between military leadership, parents, local schools, and communities. Educates school administrators, counselors and educators on the military lifestyle and how it impacts the military connected child, addresses concerns between military families and schools, information on educational options for K-12 children (special needs schools, home school, charter school, private school).
